Cygnet Hospital Harrogate launches discharge planning pilot

We are excited to announce that Cygnet Hospital Harrogate has launched a six month pilot of a revised version of the successful Planning Appropriate Discharge (PAD) process.

The pilot commenced on the 1st September 2016 with a number of open days for key NHS stakeholders with the aim of sharing best practice and creating synergy with stakeholder approaches.

PAD offers a robust, multi-disciplinary process which allows for an appropriate plan for discharge to be agreed based upon the early identification of complex concerns which would ordinarily create unnecessary delays to discharge. Each service user is central to the success of PAD and a proposed date of discharge based upon service user’s identified needs is established as early in their admission as is possible with action plans required in order to meet this deadline.

Cygnet Hospital Harrogate will be running the pilot with a goal of improving our existing communications with commissioners while building close links with community services thus ensuring appropriate input for our service users when approaching their discharge.
