The Fields is a 54 bed specialist residential home for young people and adults with learning disabilities and associated complex needs. The home is made up of seven separate units. We pride ourselves on a warm and welcoming environment, creating bespoke residential packages for people with learning disabilities. These packages can be long term placements, or part of a specific commissioned outcome, for example preparing someone for supported living. As a team we work hard to provide a personalised person centred service for adults with learning disabilities in all aspects of their lives constantly working to promote choice, self-determination and independence. Through our nursing teams we ensure that people’s health and well-being is met working with individuals to promote healthy lifestyles, facilitate GP and hospital appointments, health action plans and hospital passports trying to ensure people have as much control as possible. Every individual who lives at The Fields has access to their own named nurse should they need one. We have a comprehensive advocacy package with East Midlands Advocacy Alliance which supports a whole host of advocacy led initiatives, which ensures we get independent feedback to the services we provide and provides a voice for those who find it difficult to communicate. Local area information Being on the edge of the city, the Peak District National Park and a whole host of country parks, there is never a shortage of interesting activities for people to be involved with. Having a specialist day service attached to the service (with a hydrotherapy pool and sensory rooms to name a few), we can always offer meaningful activities to people.
