Case Study: Janet Moves On

Following a time in a hospital where Janet* had initially received 1:1 support and displayed relatively high risk behaviours, the time had come to move on. Janet moved to Ducks Halt and spent time in the homely environment, moving positively through a person centred care plan with greater access to the community.

With support from the staff, Janet’s personal care and decision making processes improved. Janet’s confidence increased along with new skills, allowing her greater independence.

Janet moved on from Ducks Halt to a new flat and is now enjoying new independence and new friends.

*Name has been changed to protect Janet’s identity.
