Introduction

We work in partnership with a Registered Provider, which means that the client has an individual tenancy, allowing them their own private accommodation in a house with access to appropriate support, paid for through housing benefit.  We provide well-matched staff to support the individual through their recovery journey, hopefully reducing the level of support until the individual achieves a life which is as independent as possible. 

The level of care provided is tailor made for the individual’s needs – we can offer supported living from 24 hours per day down to a few hours per week.  Alternatively we can offer outreach to individuals who may be living completely independently or with family or carers. 

Supported living and outreach meet the individualised and specific care needs of an individual through person-centred planning, delivered through structured timetables to aid with activities of daily living and the hobbies and interests of the individual, placement plans and risk assessments.  We provide full liaison with external health and social care services i.e. CMHT, psychologists, psychiatrists, CPN's, and Social Services so that the care delivered to the individual is fully integrated.
