Our Outreach Service support's adults in their own homes and the community. Outreach works to the same principles of Supported Living, developing the individual's daily living skills and promoting independence, supporting each individual to work towards their goals. This includes improving access to the community, social and leisure activities, and accessing further education and employment. Our Outreach Service supports individuals with autism/Asperger's syndrome, learning disability, mental health problems and/or behaviours which challenge.
Location
We are a leading provider of care and support services for adults and young people from the age 16 years and above. Our service is for people with learning disabilities, specialising in autism, Asperger's syndrome, and associated conditions. The aim of the service is to support service users to manage their condition and to help each individual to achieve their optimal level of independence to enable them to live as independently as they are able. Many of our service users move on to supported living in the wider community.
