Community Rehabilitation

Cygnet have created a number of small registered hospitals in the community to assist in the gradual transference of care from a secure setting back into the community.

Our Community Rehabilitation or locked mental health rehabilitation service provides an extremely affordable solution for helping patients prepare for independent living outside of a hospital setting. Our units provide an intensive rehabilitation service that includes psychotherapy, in a community setting.

Each unit is based close to an existing Cygnet hospital to ensure that patients can move down the recovery pathway as quickly as possible. The units are Care Quality Commission registered with single en suite facilities through out.

Several Specialist Commissioning Groups report a significant increase in the need for low secure provision. This increase in need can be attributed to a number of factors but one is the lack of adequate strategies to move patients into more appropriate and lower levels of secure care appropriate to their needs.

Community Mental Health lockable Rehabilitation units is an option to be considered. These units are designed to move patients from a secure setting into the community in a safe and structured way.

Our Care Pathway

Treatment

The structured therapeutic environment and single en-suite rooms assist patients stepping down from a low secure or acute setting. Assessments can be arranged in the patient's locality and an individual treatment programme will developed upon admission.

Cygnet uses the latest evidence-based practice, based on NICE and Department of Health guidelines, to achieve the best possible clinical and social outcomes for our clients.

We have close links with local community services and encourage patients to participate in community activities wherever possible. These may include education, employment, social and therapeutic activities.

Our comprehensive multi disciplinary teams include:

  • A consultant psychiatrist
  • Junior doctor support
  • 24 hour waking nursing cover
  • Psychologists
  • Social support worker
  • Occupational therapy team
  • Social worker

We use a whole system's approach to recovery in order to maximise quality of life, independence and social inclusion, and to promote safe lifestyles. We adopt a client-centred and collaborative approach to recovery, utilising tools such as the Mental Health Recovery Star to assist clients and staff to systematically identify needs and goals and to monitor progress.

Future Developments

Cygnet has a number of contractual arrangements to provide these services in the London area and Yorkshire. In 2010 Cygnet are undertaking to open another two of these much needed facilities (Weston super Mare and Derby) to supplement their existing low secure provision.

Cygnet are currently exploring options working with property providers to develop the next step in the care pathway namely full independent living arrangements either in house or with local partners. We have two such projects under development currently.
