Is there a cost for the assessment?

There is no cost for the assessment.

What is the assessment process?

Austen Ward – will require information such as CPA, Social Worker reports and nursing records faxing to the ward to enable a decision to be made regarding admission. Once this information is received and reviewed by the clinical team admission can occur the same day if appropriate.

Bronte Ward

For a PICU referral nursing staff will require information such as CPA, Social Worker reports and nursing records faxing to the ward to enable a decision to be made regarding admission. Once this information is received and reviewed by the clinical team admission can occur the same day if appropriate.

For a complex needs referral nursing staff will require information such as CPA, Social Worker reports and nursing records faxing to the ward to enable a decision to be made regarding an assessment of the patient. During the assessment clinical staff will have a relaxed, informal talk with the patient to gain insight into the patient’s presentation and feelings about the referral to Shelley Ward. The clinical team will also require access to the patient’s records and discuss the referral with a member of the ward staff/referral team. Following assessment the clinical team will discuss the referral and assessment with the Consultant Psychiatrist who would be responsible for the patient’s treatment on Shelley Ward. Admission, if appropriate, may be agreed within a week of assessment.

Shelley Ward

Once a referral is received by telephone or letter an assessment date will usually be arranged within 5 working days, and will be carried out by the 2 senior members of the clinical team. During the assessment clinical staff will have a relaxed, informal talk with the patient to gain insight into the patient’s presentation and feelings about the referral to Shelley Ward. The clinical team will also require access to the patient’s records and discuss the referral with a member of the ward staff/referral team. Following assessment the clinical team will discuss the referral and assessment with the Consultant Psychiatrist who would be responsible for the patient’s treatment on Shelley Ward. Admission, if appropriate, may be agreed within a week of assessment.

What happens next and what is the admission procedure?

Admissions to Austen Ward and Bronte Ward PICU services can be facilitated as soon as the agreement to admit is made by the clinical team and funding authorised.

If the patient is assessed as appropriate for Bronte Ward or Shelley Ward Complex Needs, a senior member of the clinical team will contact you by telephone to arrange a convenient date for admission and discuss any further issues with you. At the time of admission, if available, an up-to-date social worker's report is also required. If the patient is not appropriate a full explanation will be given. A comprehensive report will be posted to the referrer following assessment.

On admission, along with the Consultant Psychiatrist, patients are allocated a named nurse who will be responsible for the patients' care alongside other members of the Multi disciplinary team on the ward. Each patient's care is planned on an individual basis and the multidisciplinary team works in partnership with the patient to work out agreed goals.

Do you maintain contact with referrers during an admission?

Progress summaries are sent fortnightly or monthly to referring clinicians to update them about each patient’s care and treatment programme to facilitate discharge as soon as is appropriate.

What is the average length of stay?

The length of stay is dependent on the patient's progress and is reviewed regularly in ward rounds and Care Programme Approach (CPA) meetings to which external clinicians and carers/relatives are invited as appropriate.

What happens if a patient relapses or problems occur?

It is an expectation of our clinical team that patients will occasionally have problems during their stay on the ward at Cygnet Hospital Wyke. Referring teams will be updated on the patient’s presentation and of any current management issues.

Where do service users go after they have been at Cygnet Hospital Wyke?

Patients are discharged from Cygnet Hospital Wyke to appropriate services in their local area which were approved at the Section 117 meeting prior to discharge. Patients can also be recalled to services from the referring area if a bed becomes available.
