How do I refer?
We accept referrals on a pre-planned as well as on an emergency basis. Assessments will ordinarily take place within 48 hours of referral, and the referring hospital will receive notification of whether the patient has been accepted for admission within 24 hours of assessment.
Emergency referrals can be accepted within 4 hours of initial referral. We are also prepared to travel to where the patient is located and transport the patient back to our hospital, if necessary.
Please refer to the individual Cygnet Hospital Stevenage service pages for further details as assessment times differ for each service.
Is there a cost for the assessment?
Assessment fees are not charged at Cygnet Hospital Stevenage. Please contact Rachel Pitts-Tucker, hospital manager, for cost breakdowns for any of our services.
What is the assessment process?
In cases where a face-to-face assessment is required, we can normally facilitate this within 48 hours of initial contact. We also accept emergency admissions. In such cases, we require the relevant paperwork to be faxed to us, thereafter, a decision can be made. We are also prepared to travel to where the patient is located and transport the patient back to our hospital, if necessary.
What is the admission procedure?
On admission our patients receive a full needs assessment by the admitting nurse and a full medical assessment by the ward doctor. The initial risk assessment incorporates past and current risk and we take a solution focused approach at all times. We aim to organise the first CPA six weeks following admission which will include initial assessment and HCR-20.
Do you maintain contact with referrers during an admission?
We provide weekly clinical updates for all patients from admission. NHS professionals from the referring teams are invited to attend ward rounds at any time and would be expected to attend statutory meetings e.g. CPAs, Manager’s Hearings etc.
Where do patients go after they have been at Cygnet Hospital Stevenage?
This would depend on where the patient is within their care pathway. They could be transferred back to their home area when an appropriate placement becomes available, or they could be discharged home. They may transfer to another ward within Cygnet Hospital Stevenage or, alternatively, to another appropriate service within Cygnet Health Care.
