Information for Referrers & Doctors
Do you have any vacancies?
Our bed state changes on a daily basis, our nursing team will be able to advise you
How do I refer?
Referrals can be made by phone and fax 24 hours a day.
Psychiatric Intensive Care
Patients may be admitted to Nash Ward 24 hours per day, seven days per week. Referral paperwork is considered by the multidisciplinary team at the time of receipt and a decision will be reached within two hours.
Low Secure Services
For all three low secure wards an assessment of the patient at their current location will be arranged as soon as possible following receipt of the completed referral form, usually within 24 hours. Assessments are discussed at our multidisciplinary referrals meeting and admission is arranged as soon as practically possible.
All referrals are co-ordinated by the Administration Manager, who may be contacted on 01934 428989, or at lornamatthews@cygnethealth.co.uk. Informal discussion of potential referral is also welcomed.
Is there a cost for the assessment?
No
What is the assessment process?
Two members of our multidisciplinary team will come out to meet with the patient and discuss them with the referring team first hand. The suitability of the patient for a placement at Cygnet Hospital Kewstoke is then discussed at our referrals meeting, considering both the patient’s needs, their current and past presentation and the existing mix of patients within the ward. We will then send a written assessment report out, with our recommendations.
What happens next?
Usually we can admit the patient within a week of assessment, depending on their current circumstances, any planned CPA meetings or Tribunals which should be held prior to admission, and funding.
What is the admission procedure?
Admission, if appropriate, may be agreed within a week of assessment. At the time of admission, if available, an up-to-date social worker’s report is also required.
Do you maintain contact with referrers during an admission?
Referring teams receive regular, detailed clinical updates that report on the progress of patients. We also invite the home team to CPA and 117 meetings so that they can remain actively involved in the patient’s care.
What is the average length of stay?
On the low secure wards it is generally somewhere between a year and 18 months although this does vary. On the PICU it seems to be around 2 weeks for emergency admissions.
What happens if a patient relapses or problems occur?
If problems occur after a patient has left our service then you are welcome to contact us for advice or to re-refer them.
Where do service users go after they have been at Cygnet Hospital Kewstoke?
This depends on the patient. Many are discharged to their home address, others to community rehabilitation programmes to continue the care pathway prior to returning to the community.
Would it be possible to arrange a visit to the unit?
Of course! Visitors are welcome to look around the unit and meet members of the MDT. Please contact Lorna Matthews, Administration Manager, or Bronwen Riordan, Hospital Manager, on 01934 428989 to arrange your visit.
