How do I refer?
Please call 01732 763491 to make a referral. Our lines are open 24 hours a day, 7 days per week.
Is there a cost for the assessment?
Pre-admission assessments are normally free of charge, however costs may be incurred for extended assessments or for assessments outside of Cygnet Hospital Godden Green
What is the assessment process?
- We welcome emergency referrals and aim to make the process as straightforward as possible.
- We ask that as much information as possible regarding the patient is faxed/emailed to action this promptly.
- Once an initial Risk Assessment has been completed, a member of our team will contact you to make the necessary arrangements.
- As soon as the admission is agreed, we will communicate at every stage to ensure a speedy and smooth process.
Do you maintain contact with referrers during an admission?
We maintain regular communication with the referrer, fund-holder and NHS team to meet their requirements. We believe effective communication is essential.
Where do service users go after they have been at Cygnet Hospital Godden Green?
The CPA process commences on admission to ensure solid aftercare arrangements are in place following discharge.
