How do I refer?
Meridian Ward – Male Low Secure: Refer/Assess/Report within 2 days.
Tyler Ward – Male PICU: Referral and fax information through to ward/response on acceptance within one hour/pick-up service if required.
Is there a cost for the assessment?
There is no cost for the assessment.
What is the assessment process?
Assessments are undertaken in the patient’s location and we aim to notify referrers of the outcome within 2 days.
What happens next?
Meridian Ward – Male Low Secure: the referring Trust will be contacted and a date for transfer and admission arranged.
What is the admission procedure?
On admission, patients are allocated a Consultant and a named nurse who are responsible for the patient's care. Each patient's care is planned on an individual basis and the multidisciplinary team works in partnership with the patient to work out agreed goals.
Do you maintain contact with referrers during an admission?
Weekly treatment summaries are sent to the patient's NHS hospital to enable progress to be monitored.
What is the average length of stay?
The average length of stay is dependant on the individual’s needs. Typically a patient may stay on Tyler ward for 5 weeks and on Meridian Ward for 10 months.
What happens if a patient relapses or problems occur?
If a patient relapses on Meridian Ward, we have the possibility of transferring the patient for a short stay on Tyler Ward (PICU), if necessary, and until stabilised.
Where do service users go after they have been at Cygnet Wing Blackheath?
Patients often step down to community facilities in their local area.
