How do I refer?
Is there a cost for the assessment?
There is no cost for the assessment.
What is the assessment process?
The assessment process is undertaken by members of the multi-disciplinary team (MDT), consisting of medical, nursing, and occupational therapy team members. Following assessment a detailed and comprehensive report and recommendations are sent back to the referrer.
What happens next?
For Bowling (male open acute) ward, the MDT will assess the referral using the requested documentation provided by the referring teams, and will organize admission when all the information has been received, subject to meeting the admission criteria.
For Fairfax (male complex needs) and New Dawn (Personality Disorders), following an MDT assessment the referrer would be informed if the patient was suitable for admission and arrangements discussed for transfer.
Do you maintain contact with referrers during an admission?
Progress summaries are sent fortnightly (for male PICU service and male open acute service) or monthly (for male complex needs and female personality disorders service) to referring clinicians to update them on each patient's care and treatment and regular reviews are undertaken in ward rounds and Care Programme Approach (CPA) meetings to which external clinicians and carers/relatives are invited as appropriate
What is the average length of stay?
The length of stay is dependent on the type of ward and the patient's progress. This is reviewed regularly in ward rounds and Care Programme Approach (CPA) meetings to which external clinicians and carers/relatives are invited as appropriate.
What happens if a patient relapses or problems occur?
If the patient has been discharged from the hospital, help and support will be given by the NHS team, who may re-refer if this is deemed necessary
If this occurs whilst the patient is on leave or still a patient at the hospital, the ward should be contacted to arrange return to the ward.
Where do service users go after they have been at Cygnet Hospital Bierley?
They return to their referring NHS team or a suitable placement decided at the CPA.
