Quality Measures

At Cygnet Hospital Wyke we work with commissioners to identify quality measures which serve as a framework for improving services and service user experiences within the hospital. A selection of these quality measures, including quarterly reports of our progress in attaining our goals, can be found below.

Involvement, Choice and Responsibility

Achievements in the third quarter of 2011-12 included:

  • Selected staff and service users attending the Involvement, Choice and Responsibility i4i network meetings
  • Three workshops have been facilitated on both Bronte (Female Low Secure) and Shelley (Male Low Secure) wards to ensure that both service users and staff are ready for the Shared Pathway
  • One of the staff leads also visited one of the pilot hospitals; Hazelwood House to meet both staff and service user leads to discuss how the pilot phase was developing and what changes may need to take place to deliver My Shared Pathway

We are committed to providing the highest quality of care for all its service users and recognise the need to actively involve them in their care. The NHS Plan 2010-2015 sets out a strategy to measure service user satisfaction and respond to the findings in order to improve their overall experience at all levels of care. The NHS Operating framework 2010-11 outlines the plan to improve service user experience and at Cygnet Hospital Wyke we endeavour to do this through our clinical governance systems, quality improvement drive and our commitment to developing service user involvement, choice and responsibility as an ongoing process.

Recovery Planning

Workshops were introduced to the service users and staff during community meetings on Bronte Ward and on Shelley Ward. It was explained that there would be a series of 3 initial workshops that would outline My Shared Pathway and focus on the core principles and priority areas for development. The workshops were interactive and allowed for questions and concerns to be raised and answered.

Recovery planning aims to introduce the use of recovery tools to influence traditional care planning routines so that service users can become more active and responsible for developing their own support. The emphasis is very much on joint planning.

25 Hours of Meaningful Activity

There continues to be a positive response to the changes made to the 25 hours of meaningful activity monitoring forms. All service users are exceeding the 25 hour standard.

Service users and staff from secure services across the Yorkshire and Humber region and the SCT have been meeting for a couple of years to further meaningful activity. These meetings are know as the i4i Network 25 Hours of Meaningful Activity. The spirit of the project is to develop standards for meaninful activities.

The group has worked on the definition, which has been used nationally for the 25 Hours of Meaningful Activity CQUIN: "Time spent doing activities both on your own or with others which are personal and important to you, impacts on the way you feel and which drives your treatment and recovery."

The group has also developed the bench marking tool which is also used for the same CQUIN. Service users and staff from both Cygnet Hospitals Bierley and Wyke have been involved with this group for around 18 months.

EssenCES

The Essen Climate Evaluation Schema is a questionnaire used to measure the 'social climate' of a ward. It has been identified that a ward's social climate can affect a service user’s recovery in hospital. A good social climate is when staff and service users get along, service users are engaged in therapeutic activities and feel protected and any aggressive behaviour is well contained. A poor social climate is the opposite to this, i.e. a ward that is highly volatile where service users feel unsafe and have negative interactions with staff.
