Transition Service for Young People

The Springs Community has been registered to accommodate children ages 16 and 17 years since it first opened in April 1995. Children placed at The Springs Community are accommodated under Section 20 of the Children Act. The service is able to accommodate children in our Young Persons Annexe ‘Fernleys’. Admissions to this five bedded annexe are for ages 16 to 25 on admission.

Any child accommodated in the service can be supported through transition to adulthood to ensure continuity of care and support. In-house tutors in Special Needs, Arts and Crafts and Sports and Fitness are able to continue to provide their services to children as they become young adults, therefore any programme of learning and development can continue. Any educational or vocational courses in the wider community can also continue to the next stage of learning.

As the young person progresses and reaches eighteen years of age they are able to move within the service (following risk assessment) to one of our semi-independent living units ‘Monteith Villas’. Promotion and development of independent living skills and social skills will continue to promote the highest degree of autonomy and independence for the young person. Individual plans that have been prepared and developed for the child will continue to be developed in a person-centred way working with the child, their family and care co-ordinator, through transition planning to adulthood.

A programme of development that began for the child will be supported by our staff in all areas specific to the individual as they continue to develop into adulthood. During transition these can include sexuality and personal relationships for individuals’ affected by an autism-spectrum condition, developing further awareness of their autism, epilepsy, learning disability and associated conditions. The needs, wishes, goals and aspirations of the child will be considered at all times and their safety and individuality will be paramount.
