Referrals are accepted through local authorities from all over the country for individuals who are aged between 16 and 45 years on admission.
When an interest is expressed in a referral as much information as possible is accumulated about the prospective resident, in the form of past assessments and biographical material, either in documentary or in word-of-mouth form.
This enables us to reach a provisional opinion about whether we can provide the type of services required by the prospective service user. Visits then take place.
The registered manager and a senior member of staff visits the prospective service user in his/her current environment. The prospective service user, and their relatives and care co-ordinator (seperately or together) visit The Springs Community. It is then possible for all parties to assess whether The Springs might be an appropriate placement for the individual concerned.
An initial care and support plan and terms and conditions of residence are then drawn up for long term service users and a trial assessment period, usually of three months, begins. A support plan will be drawn up for all service users, irrespective of their length of stay.
Assessment is carried out by our experienced staff team. At the end of the three months a formal review normally takes place.
Where the review finds it appropriate to end the trial period an individual care and support plan is then agreed upon.
The Springs Community offers a VFM (value for money) service with the emphasis on personalisation and positive outcomes as evidenced in our CQC report in June 2011.
This is further evidenced by the level of independence achieved by our service users and by the number of service users that move on to greater independence in the different accommodations within The Springs Community or to supported living or living independently within the wider community.
Fees are based on the needs of the individual but are around £1,350 per week (pro-rated for stays of less than a week) with additional 1:1 support charged at £11 per hour.
