Transition Service

Transition planning is important for any child and young person, particularly so for children and young people with learning disabilities who may find change of any sort difficult to deal with. It could be said that young people with autism experience continuous transition; however the transition from childhood into adulthood is particularly important.

It is a time for looking to the future, taking the leap from child services to adult services and planning to ensure that the transition is made as smoothly as possible. Generally speaking transition processes commence at age 14, however for some individuals on the autistic spectrum it can be of benefit to start thinking about possibilities for the future from a younger age.

At Cygnet Springside Stockport we work as part of a multi-disciplinary team in partnership with young people, parents/carers, educationalists and social workers in putting transition plans in place and updating this as part of a continuous process. Access to an advocate is offered to a child and the transition process is undertaken in a person-centred way; the child is the central point of the process, their views and opinions must be properly represented and appropriate communication methods utilized so that all information is accessible to them.

The transition planning process should include core areas such as further education options, housing/accommodation arrangements and potential employment opportunities; identified options should focus on the ‘ideal’ opportunities, but also what is realistic for the individual in terms of funding.
