Information about making a referral

The following provides a brief overview on how to make a referral to our different services. Referral forms can be requested from the relevant hospital or service you wish to refer to. In addition:

Acute Inpatient and PICU Services

Referrals can be made 24 hours a day directly to a hospital by phone or by faxing a completed referral form. Decisions on urgent referrals can be decided within 2 hours. With more complex referrals we undertake to make a visit and assessment within 48 hours but where possible we will endeavor to complete this within 24 hours.

Locked Services

Referrals can be made 24 hours a day directly to a hospital by phone or by faxing a completed referral form.

Assessments will be made within 48 hours of referral with a decision and written report within one week of the assessment.

Low & Medium Secure Services

A patient assessment visit can be arranged quickly with a simple phone call directly to the relevant hospital or by faxing a referral form.

Prison Transfer Service

Referrals to our Prison Transfer service can be arranged quickly with a simple phone call directly to the relevant hospital.

Adult and Children's Care and Support Services

Our services can be funded via local authority disability services, Individualised Budgets, Direct Payments, or alternatively via health or education services. They can also be funded privately by individuals and/or their families.
