Integrated Care Pathways

Care pathways are critical to delivering a recovery focused service user centred service. Cygnet Health Care prides itself in the development of pathways to suit the needs of service users. In addition to being good practice, a robust and integrated pathway also offers the best value for money which is consistent with the QIPP agenda.

We offer a wide range of services which enables us to treat service users with complex needs. For example, a service user in our female challenging behaviours unit was also able to access treatment for an eating disorder via the specialist service at Ealing. This network of services allows us to offer integrated care pathways - these are further extended by the development of Cygnet Supported Living.
