Outreach & Supported Living

We have a wide range of linked services for outreach services and supported living in the community. This allows us to put the individual at the heart of the process with a clear pathway towards recovery and eventually independent living in their own accommodation.

Cygnet Health Care works with children and adults with mental health problems, learning disabilities, autism (including Asperger's Syndrome) and adults with complex special needs.

Cygnet Supported Living can contribute to a reduction in the length of hospital stays of individuals through close monitoring by our clinical team and liaison with other health and social care agencies such as CMHT and Social Services to ensure support is in place if an individual has deterioration in their mental health. We also accept referrals of those subject to the Mental Health Act 1983 (amended in 2007) and those who are subject to a Community Treatment Order.


Supported Living

Cygnet Health Care has developed a service called Cygnet Supported Living which uses a Community Focused Recovery Pathway, for those individuals who find themselves in hospital and group home settings where accessing appropriate levels of community support, specific to their needs, has not been available or straightforward to obtain. This may mean the service user is either living in their own tenancy within supported living or on an outreach basis while living with their family or carers.

Cygnet Supported Living works in partnership with a Registered Social Landlord to provide good quality affordable housing for people who access our services. This housing is individually sourced on the needs of the individual, is paid for by housing benefits and is essentially their own tenancy. Cygnet Supported Living provide well matched staff to support the individual during their recovery pathway with the support reducing until they have reached their level of independence, which may mean the individual achieves either semi or fully independent depending on their needs and abilities.

Supported Living and Outreach achieves individualised and specific care needs of an individual through Person Centred Plans, structured timetables to aid with activities of daily living and hobbies and interests of the individual, placement plans, risk assessments, and liaison with external health and social care services i.e. CMHT, psychologists, psychiatrists, CPN's, and Social Services to name a few.

Outreach

Our Outreach Service support's children and adults in their own homes and the community and is an extension of our Supported Living service. Outreach works to the same principles of Supported Living, developing the individual's daily living skills and promoting independence, supporting each individual to work towards their goals. This includes improving access to the community, social and leisure activities, and accessing further education and employment. Our Outreach Service supports individuals with autism/Asperger's syndrome, learning disability, mental health problems and/or behaviours which challenge.
