Our supported living and outreach services are designed to meet the specific care needs of children, young people and adults with autism/Asperger's syndrome, learning disability and/or behaviours which challenge.
We provide responsive care based around the social model of disability support. Empowering the individuals we work with to have control and choice are key to the service. Support is provided in a variety of areas including daily living, social inclusion, leisure pursuits, household and finance management as well as continued education and supported employment, in order to develop and maintain the skills required for individuals to ultimately live as independently as possible.
Support is person-centred in nature and designed to be flexible around the needs of individuals and their families. Service users are supported in areas such as developing and maintaining family and peer relationships, playing a positive part in their community, making the transition into adulthood and accessing activities that they couldn't without support.
Regular monitoring and review ensures all support plans are current and allows individuals to re-assess their own goals and objectives in conjunction with their support circle. Ensuring changes are effectively communicated to all means we can provide consistency of support and are better equipped to support individuals to achieve positive outcomes.
Our supported living services are designed to be holistic in nature and as such we can also support adult service users in developing supported or independent living plans and in finding accommodation appropriate to their needs. We will work with individuals to provide tailored plans which aim to encourage independence and empower individuals, but provide support in the areas it is needed most.
