Step Forward (Locked Rehabilitation)
Cygnet Health Care provides step-forward services which aim to bridge the gap between hospital and community living, taking place within a registered hospital environment as part of the secure care pathway. We offer extensive therapy to help users enhance their daily living skills and become more independent so, ultimately, they can return home.
Our step-forward facilities accept both detained and informal service users. This includes referrals from medium and low secure services, as well as those with forensic histories. The service starts with a full risk assessment, to ascertain the appropriate levels of access and independence.
Our aim is to help users become as independent as possible, as quickly as possible. On admission, everyone receives a welcome pack to help with their daily living (which includes a dressing gown, toiletries, a voucher to spend on clothes or equipment, and so on). We hope this goes at least a little way to helping a resident feel more at home. We can also arrange day and overnight familiarisation visits for individuals prior to admission.
Once a service user is admitted, we carry out an initial assessment, looking at their strengths and needs. This includes assessing their mental and physical health, social functioning, daily living activities, communication, occupation, family dynamics and social circumstances. We also look at how aware they are of their illness and how well they are able to manage it.
With access to an extremely wide range of therapies, we develop tailor-made rehabilitation programmes in collaboration with carers and local teams. We provide regular updates to referrers and care co-ordinators, and hold an initial care programme approach meeting after six to eight weeks, and then one every three months after that.
